As our In-Home Care Nursing Coordinator, you will have the important responsibility of managing the quality of care for our clients on the Sunshine Coast. You will supervise, mentor, and train our Home Care Aides, coordinate with health care professionals and authorities, and assess new clients’ needs, matching clients with the best Care Aides for their needs/personalities, and monitor ongoing Care Plans.
Responsibilities include:
– Assessing the health needs, preferences, goals of your clients
– Developing, implementing, and monitoring individualized care plans
– Providing staff training and nursing interventions such as wound care, medication administration, vital signs monitoring, etc.
– Coordinating with other health care professionals, government agencies, Vancouver Coastal Health case managers and nurses, and family members
– Documenting and reporting on your clients’ progress and outcomes
– Ensuring the safety and comfort of your clients at all times

– An RN or LPN graduate
– Hold a valid driver’s license and access to a reliable vehicle
– Clear criminal record check and vulnerable sector check
– Excellent communication, interpersonal, and organizational skills
– Have a passion for providing client-centered and holistic care
– Up-to date Covid-19 vaccinations
